Charlotte Graham - Case Manager

Charlotte is a highly experienced Occupational Therapist, having worked within the National Health Service and private practice. She has a wealth of experience in the assessment of both children and adults with various levels of need, including multiple and complex disabilities. Her personal experience of caring for a family member with a brain injury has given her a genuine, empathetic and realistic understanding of the challenges of having, and caring for someone with, additional needs. 

Charlotte has excellent communication skills; with clients and their families as well as care teams, therapists and solicitors. She is  proactive, efficient and thorough, with excellent problem solving skills. In her spare time she is a trustee of a local charity which serves young people with special needs.

Sarah Gardner - Case Manager

Sarah is a highly specialised Occupational Therapist who has worked in both the NHS and private practice including roles in community, educational and specialist clinical services.

Sarah’s clinical skills include:

  • Holistic client centred assessments and treatment plans
  • Assessment and provision of postural management equipment and seating
  • Manual handling
  • Specialist equipment for activities of daily living
  • Assistive technology

Sarah has extensive experience of working with children and young adults with multiple and complex health needs including Cerebral Palsy, Acquired Brain Injury, Neurological Disorders, Autism and Learning Disabilities.

Sarah’s work with clients, families and carers has provided her with a deep understanding of the day to day challenges involved in coordinating care and therapeutic input for clients. Sarah’s experience of working with multi-disciplinary teams and agencies has allowed her to develop effective communication and team working skills which support her commitment to establishing the best possible care and outcomes for the individuals and families she works with.

Claire Walton - Case Manager

As a qualified Occupation Therapist, Claire says caring for others is something that comes naturally to her. Although a Specialist Pediatric Occupational Therapist, she has also worked extensively with adults of all ages with a wide and varied range of both physical and mental disabilities.

Claire’s pediatric experience includes working with children who have complex neurological and genetic disorders, supporting them and their families. Her pediatric role involves supporting children to access education, play and self-care activities.

Claire’s role as a Case Manager means she can concentrate on her clients and enable their skills and abilities to overcome their disabilities. She believes in turning negatives into positives and her first class communication skills mean she is caring, supportive and reassuring, in the process.

Angelina Gwangwaa - Case Manager

Social worker Angelina specialises in working with children with ASD, Cerebral Palsy and Brain Injury. She is passionate about helping children live a fulfilled life and helping them reach their potential.

Angelina comes from a large family whose ethos was to help and support people, especially those who are unable to do so themselves. As the mother of a child on the autistic spectrum, Angelina has a  particular empathy with her client’s parents and family.

She brings her management skills to her role as a Case Manager. Angelina has excellent people management and team building skills, and much experience in supervision, audits and case reviews. Her previous roles in Social Services includes working with vulnerable children where she was a Sign of Safety Practice Leader.

Angelina describes herself as self-motivated, highly conscientious, enthusiastic, dedicated and focused. Her calm composure and ability to work under pressure means she will rise to the most challenging of situations.

Rebecca Taylor - Nurse Advisor

As a child I always knew I wanted to be a children’s nurse. On leaving school I began my children’s nurse training at Great Ormond Street Hospital for Children in London.

After qualifying I consolidated my training on the Metabolic/Gastrointestinal Ward. I then felt I needed to gain experience in general paediatrics and moved to a District General Hospital, where I spent five years, and was promoted to Ward Sister. I returned to Great Ormond Street in 1994 as Ward Sister of the ENT Surgical ward gaining experience in airway management.

In 2003 I was sponsored to work for two years on the General Surgical ward at the Children’s Hospital in Perth, Australia. I stayed for twelve!

Over the years I have developed many skills in caring for children with a huge variety of complex needs including: tracheostomy care, enteral feeding, Total Parenteral Nutrition and Hickman Lines.

On returning to the UK I worked in the community on several complex children’s care packages, before joining the 360 team. I am now able to use my skills and expertise to assess and plan children’s complex clinical needs, and train staff within the community.

Debbie Walter - Community Assessor

Debbie brings 15 years of experience in the care sector to 360 Case Management.  Her role includes conducting and writing risk assessments, and writing and reviewing Support Worker plans for clients in the community.

Debbie’s background includes working with children and adults with clinical, physical and learning disabilities. Her work involves managing staff, working in partnership with external professionals such as clinicians, social workers, and Care Quality Commission.

Debbie is also qualified in Assessing Vocational Achievements and has an Undergraduate Certificate in Health and Social Care. Her extensive experience means she is well placed to support clients, their families, and the 360 Case Managers and other team members.

360's Training Experts

Lin Pronger - Training Manager

Lin is 360 Case Management’s Training Manager and is responsible for coordinating the complex care and mandatory training. This training is available for 360’s care staff, as well as outside organisations using this service.

Lin is a versatile and enthusiastic manager who understands the importance of excellent customer service and teamwork. She is extremely excited to be part of 360 Case Management and is looking forward to supporting clients and their families to ensure that they have the highest quality of service that 360 has to offer.

Lin’s knowledge of healthcare spans some 30 years. Her experience includes her role as a Registered Manager for a care organisation caring for adults and children with complex care needs. This includes Acquired Brain Injuries, Learning Disabilities, Clinical Care and Spinal Injuries.

Previously, Lin worked in the corporate sector as a Medical Insurance Claims Team Leader for BUPA. Following this she worked for Legal and General Healthcare, as Manager of their Private Medical Insurance Claims Division.

In these roles, Lin’s responsibilities and experience included:

  • Cost Management of exceptional/high value claims
  • Technical updates and sign-off approval for existing and future products
  • Research into innovative medical techniques in order to recommend eligibility of treatment
  • Recruitment, training and development of all Claims Assessors.

360's HR & Payrol Experts

Leanne Deakin - HR and Case Coordinator

Joining the team in March 2017 Leanne brings a wealth of experience to 360 Case Management by providing administrative HR and Payroll support on a daily basis.

Her background of working within a solicitors and achieving her Certificate in Personnel Practice lends well to supporting the families and our Case Managers.

She excels at running our weekly and monthly payrolls and is always on hand to help.

Lisa Sutton - HR and Case Coordinator

Lisa has been providing case management support to clients and HR Client Managers since 2013.

She is an integral part of the team, assisting with the recruitment process and general HR administration in a professional, positive and approachable manner.

Lisa has previously worked as a Legal Secretary and Recruitment Consultant.
