Client Centred Holistic Case Management
360 Case Management has an experienced range of Professionals including Occupational Therapists and Nurses. They aim to match the Case Manager to the individual case by profession, personality, location, and skill set. This ensures a personalised and professional service to each of our unique clients.
All 360’s Case Managers hold a relevant health qualification with a minimum of 6 years postgraduate experience.
360 Case Managers offer:
An initial needs assessment, detailing client-centred goals and Case Management recommendations. The report is fully costed with timeframes for intervention including care, medical, housing, psychological, education and work-related information.
Co-ordinate the rehabilitation programme Including the instruction and monitoring of therapy interventions, liaison with statutory agencies and specialists and facilitating multi-disciplinary meetings.
Recruitment and Management of a Care Team ensuring high quality staffing, coordinating training, supervisions, team meetings and care plans.
Assistance with Housing. Identifying appropriate housing and coordinating adaptations.
Communication and liaison with statutory services including the NHS, Social Services and Benefits Agencies.
Liaison with specialists in the legal claim with regards to appropriate housing, care and therapy provision.
